St. Peter's Health Partners is seeking a full-time Advanced Practitioner to join Palliative Care Partners at St. Peter's Hospital located in Schenectady, New York.

Palliative Care Partners is a service line across St. Peter's Health Partners that supports patients and their families who are dealing with a serious illness. The practitioner will provide palliative care services in the hospital inpatient setting. Palliative care experience preferred.

The selected candidate will work closely with an interdisciplinary team including an MD, Social Worker, and Spiritual Care support to provide comprehensive care to patients with advancing illness primarily in the hospital setting. The Advanced Practitioner will work with consultant and practice staff to manage pain and symptoms, address goals of care and treatment plans and provide guidance and education to patients and families.

  • Work schedule is Monday – Friday, 40 hours a week.
  • Preference will be given to candidates who are board certified in Hospice and Palliative Care or who have experience in palliative care.
  • The inpatient position is located at St. Peter's Hospital in Albany.
  • We provide a supportive work environment and thorough orientation.


  • Board Certification, NYS licensure and DEA required.
  • Applicants should possess excellent clinical skills, communication skills, and a strong commitment to providing excellent care that is team oriented.
