About Us:

The Northern Adelaide Local Health Network (NALHN) offers a range of primary health care services across the northern metropolitan area of Adelaide, with a focus on providing preventative and health promoting programs in the community, and transition and hospital substitution and avoidance programs targeted at chronic disease and frail aged.

The Division of Aged Care, Rehabilitation and Palliative Care is predominately based at Modbury Hospital and includes Inpatient units, In the Home Services and the Day Rehabilitation Service. The Rehabilitation programs specialises in amputee, stroke and reconditioning client groups. Each service is unique in their individual rehabilitation program, and clients often transition between multiple service areas across their continuum of care.

The Inpatient Rehabilitation Unit provides specialised multidisciplinary rehabilitation services across 3 wards in Modbury Hospital. Each newly renovated ward has its own tailored gymnasium with up-to-date facilities, and shared therapy areas link each ward to its neighbour.

The Rehabilitation in the Home program provides home-based rehabilitation services where clients are seen on a daily basis – mirroring an inpatient service setting but with the added benefit of the client being in their own home environment. Clients also have the option to receive tailored tele-rehabilitation therapy sessions to complement the home visiting service, and connect with their therapists and medical team virtually.

The Geriatrics inpatient units include Geriatric Rehabilitation and Geriatric Evaluation and Management Unit, along with Geriatrics in the Home program. All these services offer specialist comprehensive assessment and therapy from Geriatricians, Nurses, Physiotherapists, Occupational Therapists and Social Workers. Clients who are in the Geriatrics in the Home program receive between one and three home visits per day from members of the multi-disciplinary team, depending on their goals. This service is also designed to mirror an inpatient service setting, but provide specialist geriatric services in the client’s home environment.
